Greetings Creative People,

When you are ready to register for classes, please download the following required registration forms:

• Registration Form
• Business and Service Style Contract 
• Medical and Liability Release Form

These documents can be found in 'Google Docs' please click on each document, print, fill out the required forms and email or scan them to my email address: sewingtreasures2013@gmail.com

You have some options to submit the required registration forms and payment:

Option 1: Once you have completed filling out the forms, scan onto your computer (save as a PDF file please) and attach forms to your email and send completed forms to:

Please make your payment online by clicking the PayPal payment button.

Option 2: Contact me via phone and we will schedule a meeting location to receive your forms and payment in person.
